Services are provided in Nottingham and agreed areas of Nottinghamshire, subject to assessment and capacity.
How we work
We begin with the person rather than a standard package. Assessment considers routines, communication, strengths, support needs, risks, relationships and desired outcomes. The agreed plan then guides staff duties, recording, escalation and review.
