Assistance with washing, showering or bathing, including preparation, privacy, safe access and respectful support at the level required.
Support to choose clothing, dress, manage fastenings, shave, style hair or complete other personal routines.
Prompting or assistance with toileting, continence products, hygiene, changing clothes or bedding and reporting skin or health concerns.
Support with teeth, dentures and mouth care, together with observation and escalation of relevant concerns.
Support to move safely during agreed routines, using equipment and techniques set out in the care plan and risk assessment.
We ask how you prefer care to be provided, including the language you use, your usual routine, cultural or faith considerations, privacy preferences, preferred level of prompting and who may be involved in discussions. Staff should explain what they are doing, seek consent and avoid rushing or taking over tasks the person can still do.
An assessment considers the home environment, mobility, equipment, continence, skin, communication, medication, nutrition, risks and professional guidance. Where equipment, two-person assistance or clinical input is required, this must be confirmed before the task is agreed.
Care workers record the support provided and report refusals, discomfort, skin concerns, changes in mobility, confusion, pain or other meaningful changes. The care plan and risk assessment are reviewed where circumstances change, following relevant incidents and at planned review points.

Yes. Your preferences, consent, abilities and desired level of involvement form part of the assessment and care plan.
Yes, with your agreement or other lawful authority. Family knowledge can be helpful, but your own rights and wishes remain central.
Not automatically. Staff should support you at the level you need and encourage safe participation and independence.
Medication support may be included where it has been assessed, authorised and allocated to staff who are competent for the agreed task.
Your decision is respected unless there is a lawful reason to act differently. Staff record what was offered, seek to understand the reason, explain relevant risks and escalate concerns where needed.
