Use this route for current approved homecare and Personal care services. Do not use it for emergencies or development-stage supported-living, autism or learning disability specialist referrals.

Good to know before you send

After submission

You receive an immediate confirmation and referral reference. A human response is provided within four working hours during staffed hours. We may ask for clarification, arrange assessment, explain that the referral is outside scope or confirm that the referral is progressing.

Urgent discharge

Select the urgent discharge route and provide the expected date and essential information. A member of the team responds within two working hours during staffed hours. This does not guarantee mobilisation.

Security

Uploaded files are scanned, stored outside the public website and limited to authorised staff. Sensitive information is not repeated in email subject lines or ordinary acknowledgement emails.

Emergency and safeguarding

Call 999 for immediate danger or a serious medical emergency. Follow the relevant local safeguarding route for urgent protection concerns and also inform MoreLoved Care where the referral or current service is affected.

Can I save the form and return?+

Yes. Longer professional referrals should provide secure save-and-return functionality with an expiry period.

Which file formats can I upload?+

The live form should list approved formats and size limits. Files are accepted only where they pass security checks.

Can I email documents instead?+

Use the secure upload route where available. Contact the referral team if an accessible or technical alternative is required.

Will the person receive a copy?+

This depends on the professional process, lawful sharing and agreed communication. The referrer can download a submission summary that excludes system-only information.

What happens to referrals that do not proceed?+

They are retained and deleted according to the approved privacy and retention criteria, subject to legal, safeguarding or complaint requirements.
