After submission

You receive an immediate confirmation and enquiry reference. A Care Coordinator provides a human response within one working day. Urgent hospital-discharge requests follow the separate priority route.

What the response means

The first response may clarify information, arrange a conversation, explain that the request is outside current scope or invite an assessment. It does not confirm service acceptance or a start date.

Emergency information

Call 999 for immediate danger or serious illness. Use urgent safeguarding and current-client routes where applicable. This form is not continuously monitored as an emergency channel.

Can I complete the form for myself?+

The short launch form should normally be completed in one session. A longer preliminary questionnaire may provide secure save and return.
