Current care role

We can consider assessed support with Personal care, medication, nutrition, hydration, continence, mobility, daily routines, wellbeing and agreed monitoring or reporting. Clinical diagnosis, prescribing, treatment and nursing remain with the responsible health professionals.

Hospital discharge

Urgent referrals should include discharge date, assessment, medication, equipment, moving-and-handling guidance, wounds or skin information relevant to care, escalation, professional contacts and funding authority. A human response is provided within two working hours during staffed hours, but mobilisation remains subject to safe readiness.

Delegated healthcare tasks

A delegated task is considered only where delegation is lawful, the delegating professional retains appropriate responsibility, written instructions and escalation are current, training and assessment are task specific, consent is addressed and ongoing review is defined.

Medication and time-critical care

The referral should distinguish prompting, assistance, administration and any specialist task. Time-critical medication or interventions require achievable visit timing, contingency and clear missed-dose instructions.

Equipment and moving support

Hoists, slings, beds, pressure equipment and other aids must be available, suitable and supported by current instructions. Staff do not improvise transfers or use unfamiliar equipment without appropriate preparation.

Communication and reporting

We agree who receives routine records, what must be reported immediately, how changes are escalated and how package reviews will occur. Sensitive information is exchanged through approved secure channels.

Children and young people

Approved children and young people’s packages require age-appropriate communication, parental or legal arrangements, safeguarding, education or family context and clear clinical responsibility for any delegated healthcare task.

Is MoreLoved Care a nursing service?+

No. We are registered for Personal care. Nursing treatment remains with suitably qualified professionals.

Will you accept delegated clinical tasks?+

Potentially, but only after formal review of delegation, instructions, training, competency, consent, indemnity and monitoring.

Can you start before all equipment arrives?+

Only if the interim package can be delivered safely without that equipment. Essential equipment must be in place before the related task begins.

How are urgent changes reported?+

The agreed escalation route identifies the relevant health professional, commissioner, family or emergency service.

Do you support Fast Track packages?+

Fast Track or other CHC-funded requests can be considered within our current service scope and capacity. Funding status does not remove the need for safe assessment.
