Discharge can move quickly, but care still needs to be safe and clear. We consider the person’s current abilities, medication, equipment, home environment, follow-up plan and the support required from the first day home.

What discharge support may include

An agreed package may include Personal care, dressing, continence support, medication, meals and drinks, mobility, reassurance, practical settling-in tasks and monitoring of specified changes. We can also support communication with agreed family members and professionals.

Information we need

Please provide the expected discharge date, address, care assessment, medication information, moving-and-handling instructions, equipment status, relevant risks, professional contacts, funding authority and any time-critical tasks. Where information is incomplete, we may need clarification before confirming suitability.

Urgent referrals

Urgent discharge requests receive immediate automated acknowledgement and are reviewed by a member of the team within two working hours during staffed hours. This is an initial response standard, not a guarantee that care can begin within that period.

Before the first visit

We confirm access, visit times, staff skills, equipment, medication arrangements, emergency contacts, documentation and what the person expects. Care cannot begin safely where essential instructions or equipment are missing.

After care begins

An early review checks whether the package reflects the person’s needs after returning home. Changes, deterioration or concerns are communicated through the agreed route. MoreLoved Care does not replace urgent medical care, district nursing or clinical follow-up.

This depends on assessment, location, required skills, information, equipment, funding and current staffing. We do not confirm a start until safe arrangements are in place.

Yes. We will still need appropriate information and involvement from the person or authorised representatives and professionals.

We provide care and practical support. Rehabilitation programmes remain under the direction of relevant health professionals, although agreed routines may be supported within the care plan.

Transport or accompaniment may be considered only where specifically agreed, risk assessed and operationally suitable. It is not automatic.

Staff follow the agreed escalation plan. Serious or immediate concerns require 999 or other appropriate urgent health services.
